FORMS

1. Vacation Request Form - Send to Liza for approval. Total yearly balance 20 days + 2 wellness days.

2. Consult Log - Be sure to record all the news inpatient and clinic patients you see. Please send to Liza monthly. Consider saving on U-drive for easy updates at all clinical sites.

3. Conference Request Form - You'll need to get approval for academic conferences in order to use your GME stipend for expenses. If you have an abstract accepted to a conference and will be presenting, the program will pay for your registration, housing, and travel separate from your GME stipend.
